TO SEND A CASE TO CHITTUM LABORATORIES

Call us toll-free at 1.877.429.8669 to arrange for an in-office pick-up or have shipping supplies sent.

To package your case, separate your model/impressions and place them carefully between the two layers of protection foam. Tape the box closed to prevent breakage during shipment. Affix Fedex address label (see below ).

You may use 2-day delivery service with Fedex, which guarantees that your case arrives on the second business day.

Use the air bills inserted in the back of our presentation folder to ship your first 2 packages. All shipments must include a completed CHITTUM LABORATORIES prescription for each case. Additional pre-printed air bills and prescription forms will be sent to you with the return of your first case. All packages are automatically insured for $100.

Separate the receipt (left half) from the shipping label.

Peel the backing from the shipping label (right half).

Affix the shipping label to your package.

Call 1.800.Go.FedEx® for your pickup, and information regarding drop off locations conveniently located.

You may bundle (securely together) as many cases as you like and use 1 air bill.

Please allow 10 full working days. In-lab fabrication times do not include shipping days, weekends, or holidays.
RUSH SERVICE
Please arrange
and pre-schedule all Rush cases before shipping to Lab. An additional charge
will apply.
